1. Agreement to these Terms
These Terms of Use (“Terms”) govern your use of fullcirclecare.com (the “website”), operated by Leslie Peterson, N.D., P.C., a Utah professional corporation doing business as Full Circle Care (“we,” “us,” “the practice”). By using the website you agree to these Terms; if you do not agree, please do not use the website.
These Terms govern the website only. They do not govern, alter, or limit the professional relationship between the practice and its patients, which is governed by applicable law, professional standards, and the agreements made in the course of care.
2. Who we are
Full Circle Care is the naturopathic medical practice of Dr. Leslie Peterson, N.D., a naturopathic physician licensed by the State of Utah. We provide in-person care at our office in Murray, Utah, and telemedicine follow-up visits to established patients located in Utah.
3. The website is informational
The website describes our practice and services, answers common questions, and lets you request a Fit Call (Section 5). It is provided for general informational purposes.
Nothing on the website is medical advice. The content — including descriptions of services, conditions, symptoms, and approaches to care, and including the interactive symptom-awareness tool on our home page — is general information, not a diagnosis, treatment recommendation, or substitute for care from a licensed clinician who knows your situation. The symptom-awareness tool in particular is an educational feature: its responses are generic and are not an assessment of you.
Using the website does not make you a patient. No physician-patient relationship is created by browsing the website, using the symptom-awareness tool, booking or completing a Fit Call, or contacting us through the website’s links. A physician-patient relationship with Dr. Peterson is established only through the practice’s intake process and an initial visit.
4. Emergencies
The website is not for emergencies and is not monitored for urgent messages. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
5. The Fit Call
The Fit Call is a free, 15-minute telephone conversation with our Practice Manager — not with Dr. Peterson. Its purpose is to understand what you are looking for, explain how the practice works, and determine together whether the practice is a good fit. It is not a medical consultation: no diagnosis is made, no treatment is recommended, and no physician-patient relationship is created by booking or completing one. Every patient who joins the practice is cared for by Dr. Peterson herself; the Fit Call is the step before that care begins.
Booking is handled through a third-party scheduling tool (Section 7). We may reschedule or decline a Fit Call at our discretion, and booking one does not obligate the practice to accept you as a patient or obligate you to become one.
6. Who may use the website; geographic scope
The website is intended for adults. Our services are available only to patients located in Utah: Utah licensure requires that our patients be located in Utah, in-person care is provided at our Murray office, and telemedicine follow-ups are available only to established patients within Utah. The website is directed to a Utah audience and we do not target residents of other states or countries.
7. Third-party services and links
The website links to and embeds services we do not operate, including Google’s appointment-scheduling tool (booking), our patient portal operated by Charm, and map services. Those services are governed by their own terms and privacy practices; we are not responsible for their content or operation. Our Privacy Policy describes what information, if any, reaches each of them.
8. Intellectual property
The website’s content — text, photographs, graphics, design, and code — is owned by or licensed to the practice and protected by copyright and other intellectual-property laws. You may view, download, and print pages for your personal, non-commercial use. Any other reproduction, distribution, modification, or republication requires our prior written permission. “Full Circle Care” and the website’s branding may not be used without our permission.
9. Acceptable use
You agree not to: use the website for any unlawful purpose; attempt to gain unauthorized access to the website, its server, or connected systems; interfere with the website’s operation, including by introducing malware or conducting denial-of-service activity; scrape or harvest content or data by automated means; misrepresent your identity in communications with the practice; or use the booking tool to make appointments you do not intend to keep or on behalf of a person who has not authorized it.
10. Disclaimer of warranties
The website is provided “as is” and “as available,” without warranties of any kind, express or implied, including implied warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement. We do not warrant that the website will be uninterrupted, error-free, or secure, or that its content is complete or current.
This disclaimer applies to the website only. It does not disclaim, limit, or modify any duty, standard of care, or warranty arising from professional health care services the practice provides to its patients.
11. Limitation of liability
To the fullest extent permitted by law, the practice, Dr. Peterson, and the practice’s personnel will not be liable for indirect, incidental, consequential, special, or punitive damages arising from your use of, or inability to use, the website, and our total liability for claims arising from the website will not exceed one hundred dollars (US $100).
This limitation applies to the website only. Nothing in these Terms limits or excludes liability for professional health care services provided to patients, or any liability that cannot be limited or excluded under applicable law.
12. Indemnification
You agree to indemnify and hold harmless the practice and its personnel from claims, damages, and reasonable costs arising from your violation of Section 9 (acceptable use) or your infringement of intellectual-property rights in connection with the website. This obligation relates to misuse of the website only and does not apply to any matter arising from professional health care services.
13. Disputes and governing law
These Terms and any dispute arising from the website are governed by the laws of the State of Utah, without regard to conflict-of-laws rules. You and the practice agree to the exclusive jurisdiction of the state and federal courts located in Salt Lake County, Utah, for any dispute arising from the website that is not resolved informally. We each retain the right to bring qualifying claims in small claims court.
Nothing in this section applies to any claim arising from professional health care services, which is governed by the law otherwise applicable to such claims, including Utah’s prelitigation requirements for medical claims.
14. Changes to these Terms
We may revise these Terms by posting the updated version at this address with a new effective date. Material changes will be conspicuously noted on the website. Your continued use of the website after the effective date constitutes acceptance of the revised Terms.
15. General
If any provision of these Terms is held unenforceable, the remainder continues in effect. Our failure to enforce a provision is not a waiver of it. These Terms, together with the Privacy Policy, are the entire agreement between you and the practice concerning the website — and, for clarity one final time, they neither create nor alter any agreement concerning medical care.
